Required Camp Forms

The following required camp forms are completed in our registration portal and must be submitted no later than June 1st for your camper to attend camp.

To find these forms please visit http://www.congocamp.campbrainregistration.com and log in to your camp account. Then select View Details under your camper’s name and scroll to the forms section.

Required Camp Medical Forms

The following two forms must be completed and uploaded to our registration portal no later than June 1st for your camper to attend camp.

Please note that all medical form are only valid for one year from the date signed and must remain valid for the entire duration of your camper’s enrollment at camp this summer.

Additionally, please complete and email the following medical forms to the camp nurse (clinic@congocamp.org) as needed:

Proof of Identity (POI)

Virginia law requires all new campers to present certified proof of identity and age before their first day of camp. This regulation, established by the Commonwealth of Virginia, is designed to help ensure the safety and well-being of children in licensed child day programs. Campers who have previously submitted Proof of Identity for a past summer do not need to provide it again.

Congressional Camp Family Handbook

To help set campers up for a safe, fun, and successful summer, we ask all families to review the Congressional Camp Family Handbook and then complete the acknowledgment below. Your signature confirms that you have received and read the Handbook and understand the camp’s policies, procedures, and expectations.
